Request Your Form 1095-B or 1095-C
Per IRS Notice 2025-15, TSi provides full-time employees and covered individuals with the option to request a copy of their Form 1095-B or Form 1095-C, which contains information about health coverage offered to you during the reporting year.
To request a paper copy, contact us using one of the following methods:
